We keep membership simple and transparent. Here’s exactly how it works.

Standard office policy

Benefits begin upon enrollment and receipt of payment.
Missed appointments or late cancellations may result in standard missed appointment fees.
Outstanding balances: membership benefits may be suspended while an account remains delinquent.
Benefits are valid only with providers practicing at this office.
Membership automatically renews annually unless cancelled before the renewal date.

Membership is a direct plan with our practice — there are no claim forms, deductibles, or annual maximums.
Included preventive services are provided at no additional charge during your membership year.
Member discounts require payment in full at the time of service.
Discounts cannot be combined with insurance adjustments or other promotional offers.

Not an insurance plan

This program is a direct healthcare membership plan solely for care delivered at this specific dental practice. It does not satisfy minimum essential coverage mandates under the ACA, nor does it transfer to any other dental office or specialist. This membership is not regulated as dental insurance and is valid only for services provided at this practice.

Non-transferable & non-refundable

Membership fees are valid for 12 months from the enrollment date. Unused cleanings or benefits do not roll over to the next year and cannot be transferred to other family members. Membership fees are generally non-refundable after preventive services have been provided. Requests for cancellation before benefits have been used will be reviewed on a case-by-case basis.

Payment of procedures

To receive the designated 10% to 20% discount on restorative or cosmetic work, payment must be made in full at the time of service. Membership discounts apply only to payments made directly to the practice. Procedures financed through third-party financing programs may be subject to modified membership pricing.

Discount calculation

Discounts are calculated from the practice’s current standard fee schedule and may not be combined with insurance contractual adjustments or other promotional offers.

Exclusions

Discounts do not apply to over-the-counter retail products (electric toothbrushes, special rinses) or services referred out to an external oral surgeon, endodontist, or periodontist.

This is not dental insurance. It is a membership plan for care delivered at Tustin Family Dentist only. It does not satisfy ACA minimum essential coverage requirements and cannot be used at another dental office or with a specialist.
